Are you eligible to participate in this study?

You may be eligible for this study if you meet the following criteria:

• Conditions:
  Acute Myocardial Infarction
• Age: Between 70 years - 110 years
• Gender: Male or Female

• Other Inclusion Criteria:
  

  1. Subjects currently hospitalized for AMI, PCI, or CABG or hospitalized for AMI, PCI or CABG within prior 2 weeks
  2. Subjects must be competent to give informed consent
  3. Subjects understand and are able to perform study procedures

  
  

You may not be eligible for this study if the following are true:

• 1. Subjects who are non-ambulatory
  2. Subjects with moderate or severe cognitive impairment
  3. Subjects unable/unwilling to consent
  4. Subjects with PCI-related groin hematoma that precludes brisk walking
  5. Subjects who are incarcerated
  6. Subjects unable to use mHealth software in English or Spanish.
  7. Subjects with severe osteoarthritis, or joint replacement within last 3 month
  8. Subjects with Parkinson’s disease or other progressive movement disorder
  9. Subjects who regularly use walkers for ambulation
  10. Subjects projected life expectancy <3 months.
  11. Subjects who may raise concerns of safety or nonadherence issues
  12. Subjects admitted from long-term care facility
  13. Subjects who are currently listed for heart transplant
  14. Subjects are left ventricular assist device recipient
  15. Subjects who have completed ambulatory cardiac rehabilitation program within prior 3 months
