Are you eligible to participate in this study?

You may be eligible for this study if you meet the following criteria:

Conditions:
Covid-19
Age: Between 18 - 89 Years
Gender: Male or Female

Other Inclusion Criteria:
1. Participants who identifies as English or Spanish-speaking Black or Latinx
2. Has had either an appointment with a physician at one of the FHCs or through an affiliated site
3. Has any of the symptoms, signs, laboratory tests, and other indicators of infection that are associated with COVID-1981 documented in their clinic medical record since March 2020

You may not be eligible for this study if the following are true:

1. Participants who are unable to comply with the study protocol
2. Have significant psychiatric comorbidity or reports of substance abuse (as documented in the EHR)
