Are you eligible to participate in this study?

You may be eligible for this study if you meet the following criteria:

• Conditions:
  Social Risk
• Age: Between 18 years - 90 years
• Gender: Male or Female

• Other Inclusion Criteria:
  

  1. Participants who able to speak and read in English or Spanish
  2. Participants who are receiving care at the FHC
  3. Screened positive for social risk factors in the 6 months
  4. Have answered questions about things such as your housing needs, ability to obtain food, and pay for medical care at the FHC
  5. Are willing and able to provide consent

  
  

You may not be eligible for this study if the following are true:

• 1. Refuse to and/or lack capacity provide informed consent
