Are you eligible to participate in this study?

You may be eligible for this study if you meet the following criteria:

• Conditions:
  Hypertension
• Age: Between 21 years - 75 years
• Gender: Male or Female

• Other Inclusion Criteria:
  

  CHW Intervention

  1. Are of South Asian ethnicity
  2. Have a diagnosis of diabetes in the EHR or self-reported through community recruitment
  3. Have a diagnosis of hypertension or self-reported diagnosis through community recruitment
  4. Have uncontrolled BP reading(>130/80mmHg) in the last 6 months or at screening.

  Provider Surveys

  1. Clinicians employed by one of the participating clinic sites and have enrolled into the study

  Key Informant Interviews

  1. Must be a provider, clinic manager, or community health worker

  
  

You may not be eligible for this study if the following are true:

• CHW Intervention

  1. Pregnant
  2. Type 1 diabetes or diabetes secondary to other conditions
  3. Inability to perform unsupervised physical activity

  Provider Surveys

  1. Clinicians who are unable to complete the survey in the English language.

  Key Informant Interviews

  1. Participants who are unable to participate in the interview conducted in the English language.
