Promoting Safe Transitions from Hospital to Home for Children with Medical Complexity: A Health Literacy-Informed and Family-Centered Approach

You may be eligible for this study if you meet the following criteria:

  • Conditions:
    Parent Comprehension Of And Adherence To Discharge Instructions
  • Age: Between 18 years - 99 years
  • Gender: Male or Female
  • Other Inclusion Criteria:
    Parents:
    1. English or Spanish-speaking
    2. Parent or legal guardian and primary caregiver of child who has medical complexity who was admitted to the acute or intensive care units
    3. The individual’s child is =18 years old
    4. The individual’s child was discharged home or is expected be discharged home on =1 daily medication
    5. Willingness and ability to participate in study procedures
    Pediatricians:
    1. General Pediatricians or Pediatric Hospitalists who care for CMC in the inpatient setting
    2. Willingness and ability to participate in study procedures

You may not be eligible for this study if the following are true:

  • Parents:
    1. Caregiver <18 years old
    2. Poor visual acuity
    3. Self-reported hearing difficulty
    4. Previously enrolled
    5. The individual's child is not currently or previously admitted to these site
    Pediatricians:
    1. Does not meet the inclusion criteria
