You may be eligible for this study if you meet the following criteria:

• Conditions:
  Covid-19
• Age: Between 18 Year(s) - 100 Year(s)
• Gender: Male or Female

• Other Inclusion Criteria:
  

  1. Moderate or severe ARDS within 48 h of meeting Berlin ARDS criteria
     • Within 1 week of a known clinical insult or new or worsening respiratory symptoms
     • Bilateral opacities—not fully explained by effusions, lobar/lung collapse, or nodules
     • Respiratory failure not fully explained by cardiac failure or fluid overload. Need objective assessment (eg, echocardiography) to exclude hydrostatic edema if no risk factor present
     • PaO2/FIO2 = 200 mm Hg with PEEP = 5 cmH2O

  
  

You may not be eligible for this study if the following are true:

• 1. Received continuous mechanical ventilation >7 days
  2. Known or clinically suspected elevated intracranial pressure (>18 mmHg) necessitating strict control of PaCO2
  3. Known pregnancy
  4. Broncho-pleural fistula
  5. Severe liver disease (Child-Pugh Score =10)
  6. BMI>40 kg/m2
  7. Anticipating withdrawal of life support and/or shift to palliation as the goal of care
  8. Already receiving ECMO at time of randomization
