You may be eligible for this study if you meet the following criteria:

Conditions:
Amputation
Diabetes Or Peripheral Vascular Disease
Age: Between 18 Year(s) - 99 Year(s)
Gender: Male or Female

Other Inclusion Criteria:
1. Subjects who have limb amputees above the ankle
2. Subjects who are older than 18 years old
3. Treating physician is considering targeted muscle reinnervation (TMR)

You may not be eligible for this study if the following are true:

1. Below the age of 18
2. Multiple limb amputations
3. No access to telephone
