You may be eligible for this study if you meet the following criteria:

Conditions: Cardiovascular Disease

Age: Between 40 Year(s) - 84 Year(s)

Gender: Male or Female

Other Inclusion Criteria:
- Self-identify as Pakistani or Bangladeshi
- Are first generation immigrants
- Able to speak and read English, Urdu, Bangla, or Hindi
- Do not have known CVD
- Willing and able to provide consent
- Willing and able to participate in study procedures
You may not be eligible for this study if the following are true:

- Have physician diagnosed heart attack, stroke, heart failure, angina, atrial fibrillation, or past cardiovascular procedures or surgery
- Are in active treatment for cancer
- Have life expectancy <5 years due to serious illness
- Have plans to move out of the study region in the next 5 years
- Are pregnant
