Pilot Randomized Controlled Trial of Integrated Supportive Care with Nephrology Care

Brief description of study

This study is a pilot randomized controlled trial testing the feasibility of an integrated supportive care approach in the ambulatory setting in nephrology as well as it's effectiveness in relation to symptom burden, advance care planning, and quality of life in advanced kidney disease. We hypothesize the arm that receives integrated care will trend towards an improvement in symptom burden, quality of life, and advance care planning.
