Comparison of Anti-coagulation and anti-Platelet Therapies for Intracranial Vascular Atherostenosis

Brief description of study

The purpose of this research study is to see whether ticagrelor and aspirin or rivaroxaban and aspirin compared to clopidogrel and aspirin will have the most success in preventing another stroke, bleed in the brain, or death in participants who have already had a stroke from a narrowed brain artery. If you choose to participate in this study, you will be randomly assigned (like drawing straws) to one of three study treatments: ticagrelor and aspirin, rivaroxaban and aspirin, or clopidogrel and aspirin. You will receive Intensive Medical Therapy: treatment of your blood pressure, cholesterol, diabetes if you are diabetic, program to stop smoking if you smoke, and program for weight loss if you are obese. You will also have a mouthwash or cheek swab sample collected to look at the makeup of one of your genes.
