Disparities Elimination through Coordinated Interventions to Prevent and Control Heart and Lung Disease Risk

Are you eligible to participate in this study?

You may be eligible for this study if you meet the following criteria:

Conditions:
Hypertension
Age: Between 18 years - 85 years
Gender: Male or Female

Other Inclusion Criteria:
1. Identify as Black.
2. Have uncontrolled hypertension.
3. Had an appointment with a physician in the practice for routine non-emergent primary care in the last 12 months.

You may not be eligible for this study if the following are true:

1. You are deemed unable to comply with the study protocol.
2. You are participating in other hypertension-related clinical trials.
3. You have significant psychiatric comorbidity or reports of substance abuse.
4. You plan to discontinue care at the site within the next 12 months.
5. You are pregnant or planning to become pregnant in the next 12 months.
