Addressing Disparities in Hypertension and Reducing Racial Inequity through Engagement (Project ADHERE)
Brief description of study
This study will launch Project ADHERE at enrolled NYULH primary care practices. Enrolled patients will receive a tailored nurse and community health worker (CHW)-led program to improve blood pressure control, discrimination-related outcomes, and mental well-being. Nurses will train patients on home blood pressure monitoring and provide coaching and support on diet, physical activity and taking medications correctly. CHWs will provide group-based health education and phone-based coaching on hypertension self-management, including strategies for managing stress related to racial discrimination, as well as referrals to social services.
