Diabetic Retinopathy Public Health and Screening Questionnaire
Brief description of study
The objective of this study is to administer a phone survey to gain a deeper understanding of the subjective patients experience related to diabetic retinopathy. We want to understand what barriers exist that cause patients to not be able to follow-up with their diabetic retinopathy care.
