Lifestyle Management of Chronic Kidney Disease in Obese Diabetic Patients

Brief description of study

To improve the health of our patients, NYU Langone School of Medicine is conducting the Healthy Hearts and Kidneys study to evaluate different approaches to helping people with Type 2 Diabetes reduce risks of developing heart and kidney disease. This study involves lifestyle behaviors; no additional medications are involved. We will evaluate, compared to usual care, 3 different technology supported behavioral interventions for engaging older, obese, diabetic individuals with concurrent Chronic Kidney Disease (CKD) in these multiple behavior changes.
