i-Matter: Investigating an mHealth texting tool for embedding patient-reported data into diabetes management

Are you eligible to participate in this study?

You may be eligible for this study if you meet the following criteria:

Conditions:
Type 2 Diabetes
Age: Between 18 - 90 Years
Gender: Male or Female

Other Inclusion Criteria:
1. Subjects have a diagnosis of T2D for =6 months
2. Subjects have uncontrolled T2D defined as HbA1c >7% documented in the EHR on at least two visits in the past year
3. Subjects should be fluent in English or Spanish
4. Subjects should be willing to send/receive text messages
5. Subjects should be over the age of 18 years old

You may not be eligible for this study if the following are true:

1. Subjects refuse or are unable to provide informed consent;
2. Subjects who have acute renal failure, end stage renal disease (ESRD) or evidence of dialysis, renal transplantation, or other ESRD-related services documented in the EHR
3. Subjects have significant psychiatric comorbidity or reports of substance abuse
4. Subjects who are pregnant or planning to become pregnant within 12 months;
5. Subjects currently participate in another T2D study
6. Subjects plan to discontinue care at the clinic within the next 12 months
