Enhanced Quality in Primary Care for Elders with Diabetes and Dementia (EQUIPED-ADRD) R33 Trial

Brief description of study

The purpose of this program is to improve diabetes care for older adults and to also help people we call family caregivers or care partners. Family members often help patients take care of their diabetes. The study will examine if the new program can improve the quality of the diabetes care, and whether it can make the effort that participants put into managing their diabetes less difficult. It will test whether care based on explicit standards for the management of diabetes (DM) medical management for people with ADRD will improve patient symptoms and quality of life while maintaining expected clinical outcomes and decrease patient and caregiver management burden and improve care quality based on patient/caregiver preferences.
