Enhanced Quality in Primary Care for Elders with Diabetes and Dementia (EQUIPED-ADRD) R33 Trial

Are you eligible to participate in this study?

You may be eligible for this study if you meet the following criteria:

  • Conditions:
    Diabetes
  • Age: Between 60 years - 110 years
  • Gender: Male or Female
  • Other Inclusion Criteria:
    Patients:
    1. Subjects must receive care at one of the INT or CON clinics involved in the study
    2. Subjects must be 65 years and older
    3. Subjects must be English or Spanish speaking
    4. Subjects must have DM diagnosis
    5. Subjects must have documented cognitive impairment or an ADRD diagnosis (ICD-10 diagnosis in Epic).
    6. Subjects must have someone who is identified as a family or friend who provides caregiving assistance.
    Caregiver:
    1. Caregiver must have adequate knowledge of identified patient and/or participate in that member's healthcare decisions.
    2. Caregiver must be English or Spanish speaking
    3. Caregiver must demonstrate capacity to consent to research participation.
    4. Caregiver must be at least 21 years old.

You may not be eligible for this study if the following are true:

  • Patients:
    1. Subjects do not receive care at one of the clinics
    2. Subjects are not 65 years and older
    3. Subjects are not English or Spanish speaking
    4. Subjects do not have DM diagnosis
    5. Subjects do not have documented Cognitive impairment or an ADRD diagnosis.
    6. Subjects have end stage dementia, other terminal illness with <6 months to live, and/or is hospice eligible
    7. Subjects do not have a caregiver
    8. Subjects have hearing too poor to use telephone
    Caregivers:
    1. Subjects do not have adequate knowledge of identified patient and/or does not participate in that member's healthcare decisions.
    2. Subjects are under 21 years and older
    3. Subjects are not English or Spanish speaking
    4. Subjects lacks capacity to consent to research
    5. Subjects have hearing too poor to use telephone.
