Enhanced Quality in Primary Care for Elders with Diabetes and Dementia (EQUIPED-ADRD) R33 Trial
Are you eligible to participate in this study?
You may be eligible for this study if you meet the following criteria:

Conditions: Diabetes

Age: Between 60 - 110 Years

Gender: Male or Female

Other Inclusion Criteria:
Patients:- Subjects must receive care at one of the INT or CON clinics involved in the study
- Subjects must be 65 years and older
- Subjects must be English or Spanish speaking
- Subjects must have DM diagnosis
- Subjects must have documented cognitive impairment or an ADRD diagnosis (ICD-10 diagnosis in Epic).
- Subjects must have someone who is identified as a family or friend who provides caregiving assistance.
- Caregiver must have adequate knowledge of identified patient and/or participate in that member's healthcare decisions.
- Caregiver must be English or Spanish speaking
- Caregiver must demonstrate capacity to consent to research participation.
- Caregiver must be at least 21 years old.
You may not be eligible for this study if the following are true:

Patients:
- Subjects do not receive care at one of the clinics
- Subjects are not 65 years and older
- Subjects are not English or Spanish speaking
- Subjects do not have DM diagnosis
- Subjects do not have documented Cognitive impairment or an ADRD diagnosis.
- Subjects have end stage dementia, other terminal illness with <6 months to live, and/or is hospice eligible
- Subjects do not have a caregiver
- Subjects have hearing too poor to use telephone
- Subjects do not have adequate knowledge of identified patient and/or does not participate in that member's healthcare decisions.
- Subjects are under 21 years and older
- Subjects are not English or Spanish speaking
- Subjects lacks capacity to consent to research
- Subjects have hearing too poor to use telephone.
